       Bone graft failure can occur for various reasons. The body has to accept the grafting material and blood vessels must grow into it so that living bone can form. With success, the body seeds the graft material with living bone cells that grow and make everything needed to form real bone. It can occur with patients who smoke or have certain types of medical problems. These patients have decreased blood circulation that can compromise the nourishment of the graft. Many patients promise to stop smoking two weeks before the graft and three weeks after, but when actually faced with doing it, they can't.

First of all, bone graft failure can happen if the grafting material does not remain in place without any movement. Even micro-movement can severe the delicate new blood vessels growing into the graft. Without blood, there is no nourishment and the cells in the graft will die. Accidental pressure on the top or side of the graft from a removable partial denture or from muscle pull can irritate the graft site and cause movement. Any pressure on the graft will cause it to dissolve.

Bone graft failure can happen if the graft is not completely protected from the mouth and oral fluids. Bacterial naturally live in the human mouth and they can enter the bone through an opening in the soft tissue and infect the graft. Sometime antibiotics will help, but unfortunately, once infected, the prognosis is not good.
