
The majority of all root canals are successful and are finished without any root canal complication. However, a dentist will sometimes begin a root canal with the hope that the tooth decay which killed the tooth has not extensively damaged the crown and roots. Despite the good intentions, upon entering the tooth and inspection, extensive tooth decay can be a difficulty that forces the dentist to change the plan to a dental extraction.
Dead teeth are brittle and a root canal complication is that a vertical root fracture can occur during the filing procedure. The fractured root can be removed while the rest of the roots remain and are used to restore the tooth. With continued trauma the canals going into the root canals can close down so small that they can't be entered or even be seen.. This is a complication that can often be overcome but unfortunately, not in all cases.
The most common root canal complication involves the accidental breaking of the files which are used to clean and shape the root canal. If a file does break, the broken piece can often be removed. If it cannot be recovered, the broken file fortunately often seals the root well enough for the root canal to be successful. Another problem is when an endodontic instrument accidentally goes though a root. This is called root perforation. In some cases repair material can be placed in the area of the perforation and the surrounding bone will heal but the success of this procedure is not insured. The floor of the pulp chamber in the crown can also be accidentally perforated and this is called a furcal perforation.
One more root canal complication is when a prepared root is not filled properly. This can be in the form of an under-fill or over-fill. A common problem is when a patient does not return to have the root canal completed. This may cause a tooth abscess to form and develop into a serious problem. Occassionally, the body will attack a completed root canal and cause root resorption. Patient with complications often have root canal pain and in these cases, it is very important to return to the dentist for a thorough evaluation.
