TMJ hearing loss is a common symptom of TMJ patients. The ear and the jaw joint are physically right next to each other and they both develop from the same place embryologically. The pain in the jaw joint often causes contraction of the little muscles inside the inner ear that hold the malleus, incus and stapes (ear bones) in place. The contraction and spasm of these inner ear muscles causes loss of hearing since these three little bones have to move for someone to hear well.
Many TMD patients with TMJ hearing loss improve their hearing after six weeks of TMJ mouth guard therapy. It's not guaranteed, but it happens a great many of the times. All of the chewing muscles refer pain to the ear and this can also be a contributing factor. With relaxation of the masticatory muscles, the muscles inside the ear usually relax and can then function well. The soft guard supports the jaw joints so the muscles can relax and this allows everything in the area of the joint to relax, including the muscles inside the ear.
Whatever the reason for the inner ear muscles relaxing, it's almost always a good thing and contributes to eliminating TMJ hearing loss. Another muscle adjacent to the inner ear is the sternocleidomastoid muscle which attaches just below the ear. This neck muscle is almost always involve in patients with TMJ disorder. The sternocleidomastoid can refer pain into the ear and also can cause significant dizziness since this large muscle helps the head balance and orient itself.
