
TMJ treatment should be based upon the type of TMJ problem that a patient has. The jaw joint is unstable in most TMJ disorder patients and there is almost always protective muscle splinting and spasm in these patients. The jaw muscles attempt to control the problem but they eventually become painful in their attempt to control the instability. In fact, about ninety percent of all TMJ patients are most disturbed by the TMJ pain that is cause by these muscles.
The first TMJ treatment goal should be to reduce or eliminate jaw muscle pain, especially since most patients seek help because of muscle pain. A soft decompression appliance with a six week TMJ program usually helps in 1 to 2 weeks. When the muscles have relaxed and become pain free, the neck and postural problems are addressed.The appliance alone does not work, patients must also make life style changes for a more healthy life. This involves eliminating or reducing harmful foods and making postural changes at work and/or home.
Most temporomandibular joint dysfunction patients have bad bites. This means when a patient bites, the bad bite forces one or both jaw joints into a painful position. Relaxing the muscles helps, but some patients need more. There are many different hard appliances that reposition the lower jaw and joints to fit properly into the upper jaw. Which TMJ appliance is best and which provides the best TMJ treatment is a matter of debate. This is why it is very confusing for many patients who see different dentists. It is not uncommon for a patient to see several dentists and to wear several appliances before being able to control the problem.
The human TMJs are amazingly able to heal and adapt. Our practice has only recommended TMJ surgery in two patients two decades and has seen many surgical failures. With this risk, it makes sense to exhaust all nonsurgical TMJ treatment before performing irreversible treatment.
