          The cause of tooth stain varies in different cases. Not all discolorations are caused by the same thing and unfortunately, many patients really do not understand this. They just want their teeth whitened quickly and simply! However, it can not always be done successfully with just one method of tooth whitening. Each patient should be examined and alternatives discussed.

          A black tooth stain has a high content of iron or iron salts along with high concentrations of calcium in dental plaque. Blackened plaque is unusual and tends to be harmless and doesn't promote dental decay. White tooth stain can be due either to too much or too little calcium in the enamel. Decalcification is when calcium is lost dues to a bacterial acid attack. When this attack continues too long it results in tooth decay.

          Many antibiotics cause internal tooth stain when they are taken as the teeth are developing. Tetracycline was often used for acne and dental infections. It is especially good for gum disease but it can get into tooth enamel and dentin and cause a yellow color. Erythromycin used to be another common dental antibiotic but it is often responsible for giving a patient's teeth a light gray color.

          Unfortunately, no tooth whitening procedure can whiten all teeth and remove every discoloration. Crowns and many dental fillings do not change color can stand out after bleaching. Large silver fillings can be seen through enamel and give a gray look to the enamel. Bonding or tooth colored fillings that don't change color on front teeth may also stand out.
